Management of Cardiogenic Shock Complicating Acute Myocardial Infarction

چکیده
The incidence of cardiogenic shock in community studies has not decreased significantly over time. Despite decreasing mortality rates associated with increasing utilisation of revascularisation, shock remains the leading cause of death for patients hospitalised with acute myocardial infarction (MI). Although shock often develops early after MI onset, it is typically not diagnosed on hospital presentation. Failure to recognise early haemodynamic compromise and the increased early use of hypotension inducing treatments may explain this observation. Recently, a randomised trial has demonstrated that early revascularisation reduces six and 12 month mortality. 2 The current American College of Cardiology/American Heart Association (ACC/AHA) guidelines recommend the adoption of an early revascularisation strategy for patients < 75 years of age with cardiogenic shock. In this article, we review the incidence, aetiology, prevention, and recognition of shock, as well as its management.

Temporal trends in cardiogenic shock complicating acute myocardial infarction.
BACKGROUND Limited information is available on trends in the incidence of and mortality due to cardiogenic shock complicating acute myocardial infarction. We studied the incidence of cardiogenic shock complicating acute myocardial infarction and in-hospital death rates among patients with this condition in a single community from 1975 through 1997. METHODS We conducted an observational study ...

BACKGROUND Limited information is available about potentially changing and contemporary trends in the incidence and hospital death rates of cardiogenic shock complicating acute myocardial infarction. The objectives of our study were to examine 3-decade-long trends (1975 to 2005) in the incidence rates of cardiogenic shock complicating acute myocardial infarction, patient characteristics and tre...

The treatment of cardiogenic shock is the ultimate challenge of our ability to manage patients presenting with acute myocardial infarction. Despite advances in the treatment of infarcts with thrombolysis, there has been no significant decrease in the incidence of cardiogenic shock, which has remained at 7–10% during the last 20 years. Hospital mortality was over 90% in the 1970s and is still hi...

Cardiogenic shock complicating acute myocardial infarction (MI) carries a high mortality which in some series prior to 1980 exceeded 80%. Neither the use of inotropic and vasopressor agents nor intraaortic balloon counterpulsation was found to improve survival in this group of patients. Intravenous thrombolytic agents improve survival in patients with acute MI, but their role in cardiogenic sho...

Acute myocardial infarction is one of the 10 leading reasons for admission to adult critical care units. In-hospital mortality for this condition has remained static in recent years, and this is related primarily to the development of cardiogenic shock. Recent advances in reperfusion therapies have had little impact on the mortality of cardiogenic shock. This may be attributable to the underuti...
